Enrollment Checklist

• Please provide name, email, and phone number
• Birth Certificate
• Immunization records
• Unofficial Transcript
• Proof of Residency
• Proof of age
• Any Court ordered documents
• Copy of IEP/504 Plan 

For ONLINE ENROLLMENT–

• The application takes about 20 minutes
• Create a new account when enrolling ( You will receive an email confirmation, make sure to check your spam folder )
• Save your work as you go
• Have your required documents already uploaded to the computer
• You may enroll your student while you gather and turn in the necessary documents
